This form allows you (the fellowshipping member) to name a person that will notify and give COTBOC authorization to release an official public announcement of your transition in the event of your passing. It also allows you to list an emergency contact that should be notified in the event there is a life-threating situation that takes place while away from your loved ones during a COTBOC function. Upon completion of this form please notify this person providing them the following contact information:

You can also exercise the right to waive any such official announcement; in this event, make the appropriate selection in the following waiver statement thus you would not have to list anyone to contact COTBOC in the event of your passing. You must still however provide an emergency contact that we can call if you experience a life-threatening situation while in attendance of a COTBOC function.
